12/26/2023 EnchantaVaughn Approved (AM) Upon arrival tp the Winter Program, client was observed settling into class and finishing his morning snack. Client was in a calm mood, as evidenced by his quiet and relaxed expressions. Client was seen starting his morning assignment and waiting for teacher to give instructions. Client expressed good communication verbally and nonverbally and was supported for his cooperation toward peers and authority. Client was able to complete his work and turn it in on time. Client was observed helping his teacher organize the classroom and clean up trash. Client was praised and encouraged to keep up the good work. Client transitioned smoothly to lunch. No redirection was needed at this time. (PM) Client demonstrated good manners to staff by being courteous and respectful (e.g. holding the door for others, waiting his turn, and saying please and thank you). Client was monitored in his seat and conversing politely with his peers. Client displayed appropriate social behaviors until it was time to head back to class. Client decided he was going to walk away from his class without asking for permission. Teacher reported she had no idea where he was. Client lied and said he was going to see his counselor. Client was directed to return to his designated area and he refused, giving authority figures a hard time. Client exhibited violent and unfavorable behaviors by swinging and pulling away when mental health professional tried to redirect him. Client became upset and frustrated and kicked everyone standing in his way. Client was observed lashing out at others and screaming “I want to go back to class”. Client did not understand why he was being taken to the office and proceeded to show a disrespectful attitude by talking back to adults, punching, and slamming walls and other objects in his way. Client had to be restrained by administrative and TDT staff. It took client 15-20 minutes to calm down. Client was able to decrease defiant and impulsive behaviors and engage in one on one with male counselor. (EOD) As the client prepared for early dismissal, mom was contacted regarding the incident that took place and the consequences that would follow. Client was escorted home by Safety and Security due to mom not being able to pick him up. Client was cooperative but seemed very upset and uncaring towards his punishment. Client did not meet any objectives today, as evidenced and reported.
